Subject: Important 2025 Tax Form 1095-C Available Upon Request
Dear HEB ISD Employees,
We would like to inform you of an important update regarding Form 1095-C.
Beginning this year, 1095-C forms will no longer be automatically mailed to employees. Instead, your form will be available upon request and can be provided to you by mail or secure email.
What is Form 1095-C?
Form 1095-C is a tax document required under the Affordable Care Act (ACA). It provides information about the health insurance coverage offered to you by HEB ISD, including details about eligibility, affordability, and the months you were offered coverage.
What do I do with Form 1095-C?
You should keep the form with your tax records. You are not required to submit Form 1095-C with your federal tax return; however, it may assist you or your tax preparer when completing your return or verifying health coverage.
How do I request a copy of my 1095-C?
If you would like to receive a copy of your 1095-C form, please complete the Request Form by October 15, 2026. Forms will be furnished within 30 days of receiving your request.
