Immunization Records Request

The request form linked below authorizes HEB ISD Health Services to provide a copy of immunization records.  Immunization records are available for former/current students with a birth date after 1997.  Any immunization records on file will be emailed; former students over 18 must request their own records.  If you are requesting records during the school year for a current student please contact their home campus.
