Medication Policy / Guidelines

Policy

HEB ISD employees will administer prescription and nonprescription medications to students provided by a
parent or legal guardian after receiving the completed Physician’s Request for Medication Administration and
properly labeled medication. Parents should use every effort to have medication administration set for time
periods other than school hours to avoid disruption of the student’s school day.

Goal

The goal of HEB ISD is to provide continuity and safe administration of medication needed by students as
requested by the Doctor and parent when the medication cannot be given at home.

Procedure

HEB ISD requires compliance with the following regulations in regard to the administration of medication by
school personnel.

  1. Administration of Medication
    • A written, signed request containing student’s name, name of medication, dosage, time and
      route of administration and duration of medication is provided by the parent or legal guardian
      and signed by the physician. This applies to all prescription and over-the-counter medications.
    • The first dose of any NEW medication must be administered by the parent or guardian.
    • Temporary Prescription Medications (i.e. antibiotics, prescription eye drops, and temporary
      pain medication) may be administered for up to 10 days with a parent signature and discharge
      instructions from the prescribing physician or labeled prescription bottle.
    • Medication (prescription and nonprescription) must be in the original container, unexpired and
      be properly labeled. Medication in baggies or non-original containers will not be administered.
    • A properly labeled prescription medication is one with a pharmacy label that includes the
      student’s name, name of medication, dosage that matches the request form, physician’s name,
      time and method of administration, and date of prescription. The parent should request that
      the pharmacist dispense two labeled bottles of medication, one for home and one for school.
    • Herbal medications, dietary supplements, or other substances not FDA approved will not be
      administered at school unless required by the student’s IEP or 504 Plan.
    • Parent or legal guardian will notify the school nurse of any medication changes.
    • It is the responsibility of the student to report for medication administration at the designated
      time. If the medication is not given a reason will be documented and the parent will be notified
      if applicable.
    • Medications prescribed by out-of-state physicians who are registered and licensed to practice
      medicine the USA may be administered up to 30 days. After 30 days, the parent must provide a
      prescription issued by a physician licensed to practice medicine in the State of Texas.
  2. Transport/Check In
    • All medication is to be checked into the school clinic by a parent or legal guardian to be
      administered by the school nurse or authorized staff.
    • Expired medications need to be picked up by a parent or legal guardian and replaced with
      current medication.
    • End of year medications must be picked up by noon the day after school is out or it will be
      destroyed. Student medication will not be stored on campus over the summer.
  3. Location/Storage of Medications
    • All medication brought to school, including over the counter medications, shall be kept in the
      school clinic and locked medication room/cabinet.
    • Emergency medications (such as Inhalers, Epi-pens, etc.) may be carried on the student with
      permission of both the physician and school nurse. It is recommended that there be “back-up”
      medication kept in the school clinic in case a student forgets the emergency medication.
    • Epinephrine delivery devices will be stored in an easily identifiable location which will remain
      unlocked during the day and locked at night. They may be carried on the student as noted
      above.
  4. Training
    • The school nurse will train all school personnel, whom the principal has designated to
      administer medication, prior to their administration of any medication.
    • The Medication Administration Training checklist form will be filled out for each principal
      designee each school year.
  5. Field Trips
    • Any medication required on a field trip must be in the properly labeled original container or
      properly labeled unit dosage container.
    • The principal designated staff member or school nurse will administer medications on the field
      trip in accordance with HEB ISD medication procedures.
    • The parent/guardian may submit written permission for the school to waive a dose of
      medication on the field trip if the medication can be safely rescheduled or omitted.

Documentation

The nurse or designee shall record each dosage of medication administered on the student’s
individual medication record. The date, time, and signature of the person administering the
medication are required.
